Thought about this some more. Seems like a repeated cross-sectional analysis would work. That makes this a three level model: individual measurements nested in time nested in physicians. I never was able to make sense of the chains in my ivh model, so I'll return to that later. I've run a random intercept model on a binary outcome which runs to completion.
